Inform the proper organizations and authorities:

Contact any other financial institution you use, and any organization where an account has been opened in your name without your permission.

Contact a major credit bureau:

Equifax (1-800-685-1111), Experian (1-888-397-3742) or
TransUnion (1-800-888-4213), to request a fraud alert be placed on your credit report.

File a report with your local law enforcement agency, and get a case number and a copy of the report.
